Practice Clinics We Offer

Diabetic Clinic
Appointments are available with our Practice Nurse, Health Care Assistants, Nurse Associate and Advanced Practitioner for diabetic care / monitoring and support. Diabetic support is usually 6monthly.

Asthma & COPD Clinic
Our Respiratory Clinics are available to monitor the care of patients with Asthma and COPD.
We have access to Spirometry and FENO testing.
These clinics are led by our Advanced Practitioner and our Practice Nurse.

Hypertension (high blood pressure) Clinic
We offer a hypertension review clinic for patients with hypertension (raised blood pressure).
These reviews are annual and involve collection of a urine sample, BP and blood tests.
Initial appointmentsi are with our Nurse Associate our Health Care Assistants and your results are reviewed by our GP, Pharmacist or Advanced Practitioner.
Follow up is by phone call or via text message. Please ensure that your phone number is up to date at your appointment.

Coronary Heart Disease Clinic
Appointments are available weekly with the Coronary Heart Disease Nurse to monitor the care of patients with diagnosed coronary heart disease.
Our Heart Disease Nurse is called Michelle Lewis.

Dementia Care
This clinic is offered annually by our Nurse Associate, Danielle Blenkey.
Danielle has undertaken extra training to support provision of Dementia reviews for our patients.
Danielle can make referrals and signpost to carers support groups and to the GP where required.

Blood Tests
We have two Healthcare Assistants and Nurse Associate who are available morning and afternoon daily.
Blood tests are requested from our clinical team from a consultation or for monitoring of medications.
If blood tests are requested by the hospital you may be given the form to share with the healthcare professional.

Minor Operation
Minor operations are available for the removal of simple skin conditions, such as warts, minor lumps and bumps. These procedures are conducted by our GP.
We also have a joint injection clinic, also conducted by our GP.

Women's Health - HRT and Cervical Smear
HRT
We offer initial HRT after assessment and review of HRT.
This can be requested via econsult.
Cervical Smear Clinic
These are provided by our Practice Nurses and Nurse Associate.
Patients are called when their recall date is due.

Family Planning
We offer initial family planning options / advice and review of contraception. You may also be signposted to the pharmacy supported contraception service.
We do not offer implants or IUD (intrauterine device) or IUS (intrauterine system) within the practice, and would signpost you to the Teeside Together Hub if these are preferred.
Teeside Together Hub offers Sexual Health and Contraception to our patients.
With four community-based clinics and a helpline, expert clinicians are on hand to offer support to improve all areas of sexual health.
Services include clinics on both referral and drop-in basis to offer advice, sexually transmitted infection testing and to provide contraceptives.

Maternity Care
Antenatal care is provided by our attached midwife. Midwife appointments are made by the midwifery team.
The midwife will look after you during your pregnancy with regular reviews at the practice.
Booking appointments are also made by the midwife.
Our reception team will ask for information regarding the first day of your last menstrual period. This information is then passed to the midwifery team.
Our midwife is called Elizabeth Rayson

Child Development
Our GPs provide development checks on babies between six and eight weeks of age. Thereafter, the health visitor is available for all further checks and is supported by our GPs.
The Health Visitor provides health support until a child's fifth birthday, monitoring child development and growth, offering advice on feeding, sleeping, safety, promoting healthy lifestyle and safegurading vulnerable children.
Our childhood vaccination programme is delivered by our Practice Nurse.

Childhood Vaccinations
Childhood immunisations are provided by our Practice Nurse. It is important that vaccinations are given at the correct time to provide the best protection for children. If your child has missed a vaccine, please contact us to arrange catch up vaccinations.

Children Mental Practitioner
We have an attached Childrens Mental Health Practitioner who helps children from age of 4 years to 18 years.
This role is varied and can support those children with relationship difficulties, anxiety, worries and any issues that may be negatively affecting their mental health. They are able to refer and signpost to other relevant services to help children manage their mental health.
Our CHMHP is called Greg Mead

Diet, weight and Healthy Heart Checks
Weight Management appointments are available with the Practice Nurse and Nurse Associate.
Dietary Advice is available by appointment with the Practice Nurse and Nurse Associate.
Healthy Heart Checks are offered to patients from 40y and is offered every five years. This involves lifestyle screening, weight, BP recording, smoking status and blood tests. An assessment is made about a patient's risk of developing heart disease in the following 10 years.
If your risk is above 10%, we offer advice to consider review of diet / lifestyle and consider medication. Your appointment will be provided by our Nurse Associate or Health Care Assistants.
Your results are reviewed by our GP or advanced practitioner.
Follow up is by text message or telephone call. Please ensure your phone number is up to date at your appointment.

Medication Review Clinic
These are undertaken by GPs, Advanced Practitioners and our Pharmacist. These are conducted face to face or by telephone.
Our Pharmacist works remotely (not within the building), and consultations with our pharmacist are by telephone. If you are unable to take a telephone call medication review (for example if hard of hearing) then please advise our reception team.
Our main Pharmacist is called Mohammed Ramzan.

High Risk Drug Monitoring
We ‘share care’ of certain medications with our hospital colleagues.
This is called high risk drug monitoring.
This monitoring may involve regular weight, BP, ECG blood tests.
The medication taken will determine the monitoring regime.
Our administration team will contact you when these are required.

Social Prescriber
Our social prescribers connect people with community groups and services that can provide practical and emotional support.
This may include homelessness support, support with debt management, isolation, welfare benefits and low-level anxiety.
Our Social Prescriber is called Michelle Wheeler.

First Contact Practitioner
Our First contact Practitioner help patients with a range of specialist physiotherapy requirements. From assessing musculoskeltal injuries and conditions, they have vast experience in physiotherapy and provide pathways to orthopaedic treatments, back pain triage, rheumatology, radiology imaging and more.
Our FCP is called Muhammad Mubeen

Vaccination Clinics
Vaccinations are offered within the practice to at risk populations.
Vaccinations include:

- Shingles
- Flu
- Covid
- Hepatitis
- Tetanus
