Self Referral For Local Services

Most referals require a GP referral but there are are services that can be by self referral.

 

MUSCULOSKELETAL CLINIC

Patients are able to self-refer directly for triage and assessment with this service.

Read the referral information before submitting the web form, which is the preferred method of self referral.

The traige team assesses suitability for treatment.

If the MSK service is unsuitable for your needs, you will be referred back to the GP for review

 

PHYSIOTHERAPY

Physiotherapy self-referral must be completed online.

Patients registered with a GP who cannot access online forms to self refer will need to speak with a member of the practice admin team.

 

PODIATRY

Patients can self-refer into the Podiatry Service by online forms, via email and telephone, information on conditions is available at: www.southtees.nhs.uk/services/community/podiatry/

WHHELCHAIR SERVICES

Patients are able to self-refer for assessment into the South Tees Wheelchair Service via South Tees Trust website

The webform which will guide users for taking measurements for wheelchairs.

A short video explains how to take measurements for a wheelchair to aid appropriate equipment to suit individual needs.

This is for attendant propelled wheelchairs only.

MACMILLAN CANCER FAMILY HISTORY SERVICE

The Macmillan Cancer Family History Service provides a comprehemsive service for individuals concerned about their family history of cancer. They link with primary and secondaty care teams, providing specialist advice and education on current advice. They are based at James Cook University Hospital and work collaboratively with the Northern Genetics service.

NHS e-Referral Tracking

If your doctor has referred you to a specialist for further care you will be asked to book your appointment using the ‘NHS e-Referral’ system.

You will receive a letter from us within 2 weeks giving details of how to book your appointment and any passwords you may need.

To track your e-Referral you will need the following details to log into the e-Referral website:

  • Your date of birth
  • Reference number – this will be on the first page of your letter
  • Password – this will be on the second page of your letter
